What to Bring to Your Visit
A simple checklist to help you prepare for your consultation. Bringing these items helps your visit go smoothly and gives Dr. Pudhota the information she needs to give you the best care.

Before Your Appointment
Reviewing this checklist a day or two before your visit will help your check-in process go quickly and smoothly. If you have any questions, please call our office before your appointment.
Items to Bring
- Photo ID (driver's license or other government-issued ID)
- Insurance card
- Referral information, if required by your insurance
- List of current medications, including dosage and frequency
- List of allergies
- Relevant medical records
- Recent labs, imaging, or procedure reports, if available
- Completed new patient registration forms, if downloaded ahead of time
- Payment method for copay, coinsurance, deductible, or other patient responsibility
Medication Information
Please bring a complete and up-to-date list of all medications you currently take. Include prescription medications, over-the-counter medications, vitamins, and supplements. For each, list the name, dosage, and how often you take it.
Insurance and Referral Information
Some insurance plans, particularly HMO plans, require a referral from your primary care physician before you can be seen by a specialist. Please confirm with your insurance carrier whether a referral is needed.

New Patient Paperwork
If you are a new patient, you can download your new patient registration form from our Patient Resources page and bring the completed paperwork with you. This can save significant time at check-in.
Arrival Reminder
- Please arrive early enough to complete check-in before your appointment time.
- New patients may need additional time for paperwork. We recommend arriving at least 15 minutes early.
- Our practice sees patients age 18 and older.
